Admission Requirements - Freshmen:
1. OLSAT Result
2. General Weighted Average (Form 138) - signed by the Principal and marked "Graduate /Eligible for admission to College"
3. Three copies 2" x 2" recent colored pictures with white background
4. Result of the Interview with the Dean
5. Certificate of Good Moral Character
6. Certified True Copy of Birth Certificate (SECPA)
7. Police Clearance when deemed necessary
8. Medical Certificate signed by the College Physician (Nursing, Maritime Studies, Criminology).
Admission Requirements - Transferees:
1. An application for admission
2. Transfer Credentials /Honorable Dismissal from school last attended
3. Photocopy of Transcript of Records from all colleges/universities attended.
4. For Public school teachers/government employees, a permit to study from the Division Superintendent /Chief of Office is required
5. Three (3) copies of 2"x2" recent colored pictures with white background
6. Police /NBI clearance when deemed necessary.
Admission Requirements - Foreign Students:
1. Alien Certificate of Registration (ACR)
2. Student VISA
3. Official Transcript of Records in duplicate, duly authenticated by the Philippine Foreign Service Post in the student's country, point of origin or legal residence
4. Six (6) copies of Personal History Statement (PHS) forms
5. Notarized Affidavit of Support including bank statement in duplicate
6. Medical Certificate
Applicants living and/or studying outside the Philippines are considered foreign applicants.
1. Honorable Dismissal from the last school attended for transferees
2. Two (2) copies of Transcript of Records as proof of baccalaureate degree earned and of grade requirement which is at least an average rating of 85% for graduate degree and 89% for post-graduate
3. Duly-accomplished Application for Admission Form by the applicant
4. Three (3) pieces of 2"x2" ID picture and 2 pieces short brown envelope
5. Two (2) copies of birth certificate in Security Papers (NSO authenticated) and 2 copies of marriage contract (for married female students only)
6. Permit to study (for government employees only)
7. Certification of Good Moral Character from previous dean, head, or professor
8. Accomplished Recommendation Form from at least 2 persons who can vouch the competence of the applicant.
