Admission Requirements - Freshmen:
1. High School Form 137 with school’s dry seal if you are a graduating high school student, or high school graduate applicant, or Philippine Educational Placement Test (PEPT)
2. Two identical 2” x 2” photos and two identical 1” x 1” photos, with your complete name and the program applied for printed at the back of the photos.
Note: These photos should be similar to the photo attached to your application form.
3. Photocopy of any valid ID with photo
4. Photocopy of Birth Certificate
Admission Requirements - Transferees:
1. An official and original Transcript of Records (TOR) indicating date cleared by the concerned UP Unit, if you are a former UP student, OR An official TOR from the last school/college/university attended, if you are a former college student of another higher education institution
Note: An official TOR should have valid remarks. Please see Attachment A for the list of remarks that will make a TOR valid as a transfer credential.
2. Two identical 2” x 2” photos and two identical 1” x 1” photos, with your complete name and the program applied for printed at the back of the photos.
Note: These photos should be similar to the photo attached to your application form.
3. Photocopy of any valid ID with photo
4. Photocopy of Birth Certificate
1. Official Transcript of Records (1 original and 1 photocopy)
Notes:
• PhD applicants should submit an original and official TOR for both bachelor’s and master’s degrees.
• The TOR should bear the school dry seal and imprint, and the Registrar’s signature in ink, and with acceptable remarks (please see Attachment A for the list of remarks in a TOR that will make it valid as a transfer credential)
2. Two letters of recommendation from:
• Employer/supervisor
• Former professor
• Former program adviser
Note: Use UPOU Form 1a for this purpose. The form is attached to the printed application form. Or you can download it from the UPOU website.

5. Photocopy of any Government-issued ID
6. Photocopy of NSO Authenticated Birth Certificate
7. Photocopy of PRC license (not expired)/board exam rating (if applying for admission to the Master of Nursing program)
8. Photocopy of PBET/LET eligibility (for teacher applicants only)
9. Certificate of Teaching Load, UPOU Form 1b (for teacher applicants only)

This school is included under the "Universal Access to Quality Tertiary Education" law that passed in 2017.
The law covers students enrolled in State and Local universities/colleges in Bachelors and Certificate programs.
Under the Law students in accredited State and Local universities/colleges will not pay any tuition fees or misc fee.
The Law covers only first time students (students who don't already have a college degree).
However, the full implementation of this law is not clear yet.
