Admission Requirements - Freshmen:
1. UIC placement exam result
2. 2x2 colored ID picture (2 copies) for the Registrar and Admissions office
3. Good Moral Certificate
4. NSO Birth Certificate original for the Registrar
5. Report Card original
Note: Original for the Registrar's office and photocopy for the Admission
Admission Requirements - Transferees:
1. UIC placement exam result

Admission Requirements - Foreign Students:
1. 2x2 ID picture;
2. Original authenticated academic records and certificates;
3. Passport;
4. Photocopy of visa with the original for students who are holders of special visas;
5. For Philippine-born Alien Students: Alien Certificate of Registration (ACR) or Certificate of Registration (CR) and Passport;
6. For Filipinos with dual citizenship: Passports of both citizenship
REQUIREMENTS FOR APPLICATION AT THE BUREAU OF IMMIGRATION:
A. Applications for Conversion to Student Visa (18 years old and above)
1. Duly notarized letter request from the applicant.
2. General Application Form duly accomplished and notarized (BI Form No. MCL-07-01);
3. Certificate of acceptance issued by the learning institutions duly accredited by the Bureau of Immigration to accept foreign students;
4. Plain Photocopy of the applicant's passport showing applicant's bio-page, admission stamp and authorized stay of at least twenty (20) calendar days from date of filing;
5. Bureau if Immigration (BI) Clearance Certificate.
B. Applications for Special Study Permit (below 18 years old)
1. Duly notarized letter request from the applicant.
2. Duly notarized General Application Form accomplished by the applicant (BI Form No. MCL-07-01);
3. Original copy of the Notice of Acceptance (NOA) containing a clear impression of the school's official dry seal or a duly notarized written endorsement from the school for the conversion of the applicant's status signed by the school's Registrar;
4. Original copy of the Medical Certificate issued by the Bureau of Quarantine and International Health Surveillance or a government medical institution with competence to certify the applicant is not afflicted with any dangerous, contagious or loathsome disease and is mentally fit;
5. Plain Photocopy of the applicant's passport showing applicant's bio-page, admission stamp and authorized stay of at least twenty (20) calendar days from date of filing;
6. National Intelligence Coordinating Agency (NICA) clearance;
7. Bureau of Immigration (BI) Clearance Certificate.
