Admission Requirements - Freshmen:
1. Original Report Card (Form 138);
2. Birth Certificate – NSO authenticated (photocopy);
3. Certificate of Good Moral Character from the school
4. Medical Certificate for NURSING & MIDWIFERY
Admission to the College of Health Sciences:
1. Original Report Card (Form 138);
2. Birth Certificate – NSO Authenticated (photocopy);
3. Certificate of Good Moral Character (signed by the Guidance Counselor or Principal of the school he/she graduated or Barangay Clearance);
4. Laboratory results (latest Chest x-ray, CBC, urinalysis, fecalysis, HEPA screening) and Physical Examination;
5. Medical Certificate issued by the University Physician (that the student possesses good health); and
6. Result of Nursing Aptitude Test.
Admission Requirements - Transferees:
1 Certificate of Transfer of Credentials (CTC) or Certificate of Honorable Dismissal (CHD);
2 Informative copy of Transcript of Records (for evaluation purposes only);
3 Certificate of Good Moral Character;
4 Birth Certificate – NSO authenticated (photocopy).
Note: Until the Transcript of Records (TOR) is received, any action on the application is provisional. This becomes a permanent part of the University files.
Admission Requirements - Foreign Students:
1. Duly notarized letter request from the applicant 1 copy each for the following:
- B.I (Bureau of Immigration, Alien Control Officer)
- University Registrar
2. Duly notarized General Application form accomplished by the applicant (BI Form No. MCL-07-01)
3. Original copy of the Notice of Acceptance (NOA) containing a clear impression of the school’s official dry seal or duly notarized written endorsement from the school for the conversion of the applicant’s status signed by the School Registrar.
4. Original copy of Medical Certificate issued by the Bureau of Quarantine and International Health Surveillance or government medical institution with contagious or loathsome disease and is mentally fit; (Davao City, near DPWH) (Note: attach all laboratory results)
5. Plain photocopy of the applicant’s passport showing applicant’s bio-page, admission stamp and authorized stay of at least twenty (20) calendar days from the date of filing.'
6. National Intelligence Coordinating Agency (NICA) Clearance; and
7. Bureau of Immigration (BI) Clearance Certificate
8. Scholastic record in English, notarized and duly authenticated by the Philippine Foreign Post.
9. Police Clearance issued by the National Police authorities from the Student’s country of origin authenticated by the Philippine Foreign Post.
10. NBI Clearance if he/she stayed in the Philippines for more than 59 days.
11. Birth Certificate authenticated from the country of origin (red ribbon).

2. Professional Requirements
2.1 Doctor of Education (Ed.D.)
2.1.1 Must be a holder of MA in Education
2.2 Doctor in Business Administration (DBA)
2.2.1 Must be a holder of Master in Business Administration/Master in Management
2.3 Doctor in Public Administration (DPA)
2.3.1. Must be a holder of Master in Public Administration or any related course
2.4. Doctor of Philosophy in Criminology (PhD Crim)
2.4.1. Must be a holder of Master of Science in Criminology
2.5. Doctor in Management (DM)
2.5.1. Must be a holder of Master in Business Administration/Master in Management
2.6. Master in Business Administration (MBA)
2.6.1. Must be a graduate of any business program.
2.6.2. If he/she is not a graduate of any business program, he/she must have earned 12 units of management subjects or with five (5) years supervisory experience
2.7. Master of Public Administration (MPA)
2.7.1. Must be a graduate of any four (4) year program
2.8. Master of Science in Economics (ME)
2.8.1. Must be a graduate of AB or BS Economics or AB or BS Mathematics or AB or BS Statistics.
2.8.2. If he/she is not a graduate of any of the above-mentioned programs, he/she must have earned any of the following:
2.8.2.1. 6 units professional subjects in Mathematics
2.8.2.2. 6 units professional subjects in Statistics
2.8.2.3. 9 units professional subjects in Economics
2.9. Master in Library and Information Science (MLIS)
2.9.1. Must be a graduate of BS Library Science
2.10. Master in Environmental Planning (MEnvP)
2.10.1. Must be a graduate of any four (4) year program
2.11. Master in Management (MM)
2.11.1. Must be a graduate of BS Management or any related course
2.12. Master of Arts in Education (MAEd)
2.12.1. Must be a graduate of any education program
2.12.2. For Non-Education graduate
Must have earned 18 units in professional education and earned 12 units professional subjects in area of concentration.
2.13. Master of Science in Criminology (MSC)
2.13.1. Must be a graduate of any 4-year criminal education program or BS Public Safety.
2.13.2. Non-Criminology graduates can enroll provided that they are members of the Philippine National Police (PNP) and have to take the following basic criminology subjects:
2.13.2.1. Introduction to Criminology
2.13.2.2. Introduction to Criminal Justice System
2.13.2.3. Police Administration and Organization
2.13.2.4. Human Behavior and Crisis Management
2.14. Master in Social Work (MSW)
2.14.1 Must be a graduate of BS Social Work
2.15. Master of Engineering Program (MEP)
2.15.1 Must be a graduate of any Engineering Program
2.15.2 Admission of Students Enjoying Educational Benefits – Students who are grantees/beneficiaries of any college plans/scholarships can be admitted to the University provided they meet the basic admission requirements and provided further that they present duly accomplished and approved scholarship papers upon enrollment.
