Admission Requirements - Freshmen:
1. Form 138 (4th Year High School Card) PEPT (original copy)
2. Certification of Honors Received (for Valedictorian/Salutatorian /1st Honorable Mention), if any
3. Certification of Good Moral Character from the Principal or Guidance Counselor
4. Photocopy of Birth Certificate (original copy to be presented)
5. Photocopy of Marriage Contract (if married)
6. Four (4) ID Pictures (1x1) with name indicated at the back
Admission Requirements - Transferees:
1. Must have passed the qualifying exam
2. Transfer Credentials from school last attended
3. Photocopy of Birth Certificate (original copy to be presented)
4. Photocopy of Marriage Contract (if married)
5. Favorable recommendation from the President of the School or Dean of Student Affairs (school last attended)
6. List of subjects taken with grades duly certified by the Dean or Registrar for evaluation purposes
7. Interview by any member or representative of the Admission Committee
8. Students who are academically deficient (with 3 or more failures) or dismissed from other schools shall not be admitted.
Admission Requirements - Foreign Students:
1. Student Permit and Appropriate Visa
2. Alien Certificate of Registration (ACR)
3. Four (4) ID pictures (1x1) with name and course indicated at the back
For PVA, USVA, Student Awardees /PESFA Grantees. In addition to those required above the following shall be submitted:
PVA, USVA – Student Awardees – Certificate of Eligibility for Veterans Benefits
PESFA Grantees – Approved Notice of Award from the CHED /TESDA
Admission Requirements - Second Coursers:
1. Official Transcript of Records with Special Order. For graduates of accredited schools, transcript of records must bear notation that the school is accredited by a duly recognized accrediting agency and that the school is exempted from applying for Special Order
2. Photocopy of Birth Certificate (original copy to be presented)
3. Photocopy of Marriage Contract (if married)
4. Four (4) ID Pictures (1x1) with name and course indicated at the back
1. General weighted average mean of 2.0 or it's equivalent or better for the Master’s Program, and a weighted average or 1.75 or better for the Doctoral Program
2. (Applicants with lower than 2.0 weighted average in the Bachelor’s program and lower than 1.75 in Master’s Program may be admitted in a non-degree program until completion of 15-18 units. They shall qualify in the degree program if the weighted average in the first 15-18 units is 1.5 or better)
3. Official Transcript of Records (original copy), Transfer credentials and description of courses taken from other schools.
4. Duly accomplished Graduate Program application form
5. Interview with Dean /Faculty
Students in the Masters and Doctorate programs pursuing areas different from their fields of specialization in the undergraduate /master’s level respectively shall be required to take additional units in the major subjects. The Dean shall decide the number of units depending on the evaluation of the undergraduate /master’s credentials presented and on certified work experience.
