1. Two copies: 1 original; and 1 Certified true copy of TOR of BS Degree in Sciences or Arts 2. Diploma/Certificate of Graduation of Bachelor’s Degree 3. Photocopy of NSO Birth Certificate 4. Original copy of NMAT results 5. Certificate of Good Moral Character from previous school 6. Police Clearance 7. Medical Examination including laboratory results: • CBC • Urinalysis • Fecalysis • Anti-HBs • HBsAG • Chest X-ray 8. Medical/Dental Clearance from the University Clinic Placement Test Result from SPUP Guidance Center
