3. Certificate of Good Moral Character
4. Original fourth year high school card

Certificate of Physical Fitness from family physician (using SPU Manila Form)
Recent chest x-ray with interpretation from accredited clinic/hospital

3. Photocopy of Transcript of Record or copy of grades from school last attended
4. Course descriptions of subjects taken in school last attended
5. Honorable Dismissal from school last attended

Admission Requirements - Foreign Students:
Before coming to the Philippines:
Fill out the Application Form
Prepare the following:
1. Personal History Statement duly signed, with 2x2 photo. Form downloadable at www.immigration.gov.ph
2. Scholastic records authenticated by the Philippine Embassy/Consulate in the student’s country of origin or legal residence
3. Photocopy of data page of passport showing photo, date and place of birth, date of issue and expiry
4. Birth certificate or it's equivalent authenticated by the Philippine Embassy/ Consulate in the country of origin or residence
5. Two LETTERS OF RECOMMENDATION from principal, adviser, counselor, or any other official from school graduated.
6. Medical Health Certificate with life size chest x-ray and laboratory reports
