Before coming to the Philippines:
Fill out the Application Form
Prepare the following:
1. Personal History Statement duly signed, with 2x2 photo. Form downloadable at www.immigration.gov.ph
2. Scholastic records authenticated by the Philippine Embassy/Consulate in the student’s country of origin or legal residence
3. Photocopy of data page of passport showing photo, date and place of birth, date of issue and expiry
4. Birth certificate or it's equivalent authenticated by the Philippine Embassy/ Consulate in the country of origin or residence
5. Two LETTERS OF RECOMMENDATION from principal, adviser, counselor, or any other official from school graduated.
6. Medical Health Certificate with life size chest x-ray and laboratory reports
A. For Local Applicants:
1. Accomplished Application Form with 2x2 picture
2. Accomplished Student Information Sheet with 2x2 picture
3. Transcript of Records (original and photocopy)
4. NSO Birth Certificate (original and photocopy)
5. Updated PRC License (photocopy) - if applicable
6. Transfer Credentials/Honorable Dismissal/Certificate of Graduation (must indicate title of degree and date of graduation)

6. Photocopy of National Intelligence Coordinating Agency (NICA) (Apply this at the Bureau of Immigration, 3rd Flr. Main Bldg. Valid at least six (6) months from date of issuance).

8. Updated Curriculum Vitae/Resume
9. Personal Account of Academic and Career Objectives
10. Photocopy of research output and other authored publications (for PhD applicants)
11. Admission Test (to be administered by the Guidance and Testing Center)

14. Student VISA
15. ACR-I Card (pending the release of students VISA and ACR-I card, submit the photocopy of the official receipt to show that the papers are already being processed. Only upon the presentation of the student VISA and ACR-I card may the students be considered officially enrolled.
16. Satisfactory English Test Result (results will determine whether student will need supplementary English lessons before enrollment, if no English was taken previously, student will take the English Test at St. Paul University-Manila).
