Admission requirements:
I. All applicants for admission to SU College of Nursing should meet the qualifications/requirements set by the Admissions and Promotions/ Committee.
a. For First Year Students:
1. High School Average Rating of 80% and above 2. University Entrance Exam Passer – at least 50 percentile 3. Satisfactory Psychological and I.Q. Test Results 4. Personal Interview at 75% level of competence 5. Character reference from three respected members of the community 6. Evidence of physical fitness as certified by a duly licensed physician. (Those who have history and/or are presently diagnosed to have a serious communicable disease, mental illness, epilepsy, cardiac disease, hearing difficulty, obvious visual and speech defects, and other physical conditions that could hinder the performance of the nurse’s functions will not be qualified for admission.)
Addendum :
Applicants 30 years old and above, are required to undergo EKG and other laboratory exams (hemoglobin determination, fasting blood sugar, uric acid, chest x-ray) and to secure a medical certificate from SUMC. Results should be submitted before enrollment.
b. For Shiftees/Second Coursers (SU Graduates)
1. Requirement 3, 4, 5, and 6 above 2. QPA of at least 2.5.
c. For Second Coursers (non-SU Graduates)
1. Requirements 3, 4, 5, and 6 as listed 2. Evaluation of records: general average of at least 2.5 or it's equivalent 3. Validation test for subjects to be credited.
II. All applicants should submit to SUCN the following within designated dates:
a. Duly accomplished application form for admission b. Two (2) copies of 2x2 pictures taken within the last 6 months c. Application Fee to cover expenses on forms, folders, honorarium for interview, psychological test, I.Q. Test and application processing. d. Birth Certificate (just a certified photocopy). E. Self addressed stamped envelope
Tuition Fees:
P 60,000-80,000 per year

Admission Requirements - Freshmen:
1. Accomplish the online registration form. All applicants are encouraged to review the guidelines before proceeding to online enrollment: http://my.su.edu.ph/enrollment.
2. Form 138 (fourth year high school report card) indicating a general average of at least 80%. Applicants to the Nursing and Physical Therapy Programs are required a general average of at least 85%.
3. (For applicants to the Nursing and Physical Therapy Programs) Results of the Qualifying Exams.
4. Two statements of good moral character: (a) one from the previous school attended, and (b) one from a prominent member of the community.
5. One passport size (2x2) copy of latest picture.
6. Official copy of birth certificate issued by the National Statistics Office (NSO).
7. Physical examination certification issued by a registered physician attesting to the student's fitness for school. The following tests must be done at the Silliman University Medical Center: chest X-ray, dental and CBC (complete blood count). Those who are unable to undergo these tests during the enrollment period may undertake them within the first week of class.
Admission Requirements - Transferees:
1. Accomplished online registration form (read online enrollment guidelines: http://my.su.edu.ph/enrollment)
2. Transfer credentials (honorable dismissal)
3. Informative copy of academic records signed by the Registrar of the previous school
4. Two letters of recommendation: one from the previous school and one from a prominent member of the community
5. One passport size (2x2) copy of latest picture
6. Certified photocopy of birth certificate issued by the Local Civil Registrar or NSO
7. (For applicants to the College of Law, Medical School and Graduate Programs) Additional requirements from these colleges are to be accomplished separately.
Admission Requirements - Foreign Students:
1. Accomplished online registration form (read online enrollment guidelines: http://my.su.edu.ph/enrollment)
Note: There are documents listed below that the online enrollment system does not require to be uploaded as part of an international applicant's application. These documents must be mailed to the Office of the Registrar and Admissions, along with the rest of which original copies are required.
2. Six (6) copies of Personal History Statement (PHS) forms.
3. An original and a duplicate copy of transcript of records authenticated by the PFSP of the applicant's country of residence. The authenticated copies must bear the diplomatic red ribbon, original signature and seal of the PFSP abroad.
4. A notarized Affidavit of Support with proof of adequate financial support (bank statement) and Letter of Guarantee from sponsoring company/organization from abroad,
5. Birth certificate and photocopy of passport pages showing the name, picture, birth date and birthplace of the applicant
6. Two (2) letters of recommendation from:
• School last attended
• Prominent person of the community
7. Admission fee of US $100.00 in money order payable to Silliman University
Overseas students must also present their immigration papers as well as certificate attesting to their competence to do college work.
