Admission Requirements - Freshmen:
1. Passed Entrance Examination given by the College. (including aptitude test for Nursing applicants).
2. Submit to Admission Interview (or panel interview for nursing applicants) conducted by the College.
3. Good physical health attested by results of general physical health examination and laboratory examinations which includes CBS, Fecalysis, Urinalysis, and Chest X-ray. Nursing, Physical Therapy, and Medical Technology applicants must also submit Hepatitis B screening results.
4. Original Form 138 (High School Card) or it's equivalent.
5. A letter of recommendation from H.S. Principal or Guidance Counselor attesting the applicant's good moral character and fitness for college work.
6. Clear photocopy of birth certificate that is duly authenticated by the National Statistics Office (NSO).
7. I.D. Photo 2 x 2 - 4 copies, 1 x 1 - 4 copies (with red background)
8. Long ordinary mailing envelope.
9. Long brown Folder.
Admission Requirements - Transferees:
1. A letter of application addressed to the Admissions Officer.
2. Photocopy of Transcript of Records (TOR) from college (s) attended before (for evaluation purposes).
3. Has an average of at least 80% in courses/subjects taken in other school.
4. No failure in any subject related to course/program applied for.
5. Pre-exam interview by the Department Dean.
6. Passed Transferee's Examination given by the College.
7. Transfer Credentials (Honorable Dismissal).
8. Certificate of Good Moral Character.
9. Clear photocopy of birth certificate that is duly authenticated by the Ntional Statistics Office (NSO).
10. Good physical health attested by results of general physical health examination and laboratory examinations which includes CBS, Fecalysis, Urinalysis, and Chest X-ray. Nursing, Physical Therapy, and Medical Technology applicants must also submit Hepatitis B screening results.
11. Police /NARCOM Clearance.

Admission Requirements - Foreign Students:
1. Below 18 years old - Special Study Permit (SSP)
2. 18 years old and above - Student Visa
Requirements
1. Passport - original and photocopy
2. Medical Clearance from Bureau of quarantine
3. Personal History Statement with 2 x 2 photo recently taken (5 copies)
4. NBI Clearance
Other Documents (authenticated by the Philippine Embassy /Consular Office in the Student's country of origin)
1. Scholastic Records (Translated in English)
2. Police Clearance
3. Notarized Affidavit of proof of Adequate Financial Support to cover expenses for the student's accommodation and substinence as well as school fees and incidental expenses
4. Bank Certification of Deposit
