3. Official Transcript of Records for transferee or college graduate: 1 original (Registrar) and 1 photocopy (Dean)
4. Authenticated NSO Birth Certificate: 1 original (Registrar) 2 photocopy (1 for Registrar and 1 for Dean)
5. Honorable Dismissal: 1 original (Registrar) 1 photocopy (Dean)
6. 2 long folders: (1 Registrar and 1 Dean)
7. 2 fasteners: (1 Registrar and 1 Dean)
8. 1 brown envelope with plastic for the Registrar
9. 2 long mailing envelope for the Registrar
Specific Requirements:

2. Certificate of Good Moral Character from the High School Principal
3. Birth Certificate – NSO (Original and two photocopies)
4. T.O.R. For shiftees, transferees and 2nd coursers.
