Admission Requirements - Freshmen:
1. High School Card Form 138 (original and photocopy)
2. Letter of Recommendation / Certificate of Good Moral Character from High School Principal / Guidance Counselor (original and photocopy)
3. NSO certified birth certificate (original and photocopy)
4. 2 colored 1.5 x 1.5 ID pictures
Admission Requirements - Transferees:
1. Certificate of Transfer Credential / Honorable Dismissal (original and photocopy)
2. Certified True Copy of Grades (original and photocopy)
3. Letter of Recommendation / Certificate of Good Moral Character from the Dean or Guidance Counselor of the last school attended (original and photocopy)
4. NSO issued birth certificate (original and photocopy)
5. Marriage Certificate (for married female students – 2 photocopies)
6. 2 colored 1.5 x 1.5 ID picture
Admission Requirements - Foreign Students:
1. Scholastic record duly authenticated by the Philippine Foreign Service consular officer located in the student’s country of origin or legal residence;
2. Police clearance certificate issued by the National Police Authority in the student’s country of origin or residence duly authenticated by the Philippine Foreign Service consular officer. Alternatively, a certificate of good moral character from a reputable source (Employer, Government official) will be accepted.

5. Copy of passport showing photo, data & stamp of the latest arrival of the student and existing visa. Original passport must be presented for verification only.
Admission Requirements - Second Coursers:
