4. Certification of Employment and Good Moral Character
5. Two (2) pieces of 2 X 2 ID picture (recent, with name tag)
6. Grade requirement – average grade of 2.0 or better.
Requirement for Foreign Students:
1. Certificate of English Proficiency Test
2. Student Visa (9-F Visa)
3. Transcript of Records (original copy)
4. Two (2) pieces of 2 X 2 ID picture (recent, with name tag)
Note: In the event a student discontinues or drops his/her course program, the rule of dropping of course is observed to determine adjustment of fees. No official record of his/her academic performance shall be released if he/she fails to settle his/her accounts.
