Course description:
This is a 6-month course with 786 hours of lectures and 240 hours on the job training at different institutions which make the student achieve competencies leading to a productive career in health care and services. A person who has achieved this Qualification is competent to be a: • Caregiver of an infant, toddler • Caregiver of a child • Caregiver of an elderly • Caregiver of people with special needs Career Opportunities • Private Duty Caregiver • Nursing Home Caregiver • Hospital Caregiver • Clinics and Health Care • Live-in Caregiver

Admission Requirements - Freshmen:
1. Original and photocopy of report card (Form 138) or it's equivalent permanent record (Form 137)
2. Original and photocopy of certificate of good moral character duly signed by the principal
3. Certified true copy and photocopy of birth certificate
4. Three (3) copies of 2×2 I.D. Pictures
5. Two (2) long folders
Admission Requirements - Transferees:
1. Certified true copy and photocopy of birth certificate
2. Certified true copy and photocopy of marriage certificate, if married
3. Original and photocopy of certificate of transfer /honorable dismissal
4. Certified true copy and photocopy of grades from the school last attended for evaluation purposes
5. Original and photocopy of certificate of good moral character duly signed by the guidance director
6. Three (3) copies of 2×2 I.D. Pictures
7. Two (2) long folders
Admission Requirements - Foreign Students:
1. Original and photocopy of CHED certified credits earned abroad (with English translation for Koreans and Chinese)
2. Original and photocopy of verified copy of the Alien Certificate of Registration (ACR)
3. Original and photocopied Certificate of Eligibility for Admission (CEA) from the executive director of CHED
4. Two (2) photocopies of passport
5. Three (3) copies of 2×2 I.D. Pictures
6. Two (2) long folders
1. Certification of Grades (original copy)
2. Honorable Dismissal (original and photocopy)
3. Two (2) photocopies of NSO Birth Certificates
4. Photocopy of Marriage Certificate (for female, if married)

Foreign Students:
1. Academic documents authenticated by the Philippine Embassy and/or Consulate Official, located in the student’s country of origin with English translation, if written in other foreign language

3. Photocopy of student’s passport showing it's bio-page, latest arrival and authorized stay in the Philippines
4. Original and photocopy of Medical Certificate issued by the Bureau of Quarantine
5. National Intelligence Coordinating Agency (NICA) Clearance
6. Bureau of Immigration Clearance Certificate
7. Authorized letter of Bureau of Immigration
8. Two (2) photocopies of NSO birth Certificate
9. Photocopy of Marriage Certificate (for female, if married)
10. Two (2) pcs. 2×2 ID pictures
