Admission requirements:
1. Admission Test
2. Application Form c/o Graduate School Office
3. Certificate of Transfer Credentials (Honorable Dismissal) from the school last attended.
4. Transcript of Records (for evaluation purposes) from the school last attended.
5. Permit to study signed by the immediate Head of the Office.
6. 1 pc 2x2 Recent ID picture - with collar and with white background
7. Photocopy of the Marriage Contract (for married women only).
8. Two (2) letters of reference from former professors.
9. Interview with the GS Dean
10. Photocopy of NSO-Birth Certificate
11. For the Masters thesis programs and PhD programs: One-page Concept Paper on the research work for his thesis/dissertation.
