Admission Requirements - Freshmen:
1. Properly accomplished admission form
2. Four (4) 2×2 recent, identical color pictures in white background with name tag.
3. Five (5) 1×1 recent, identical color pictures in white background with name tag.
4. Submit original and photocopied Form 138 / Report Card
5. Submit original Good Moral Character certificate with dry seal and Photocopied
6. Two pieces (2) Photocopied Birth Certificate (NSO)
7. If married, two (2) photocopies of marriage certificate duly signed by a priest / minister.
8. One (1) piece Long Brown envelope
Admission Requirements - Transferees:
1. Properly accomplished admission form
2. Four (4) 2×2 recent, identical color pictures in white background with name tag.
3. Five (5) 1×1 recent, identical color pictures in white background with name tag.
4. Submit original and photocopied Form 138 / Report Card
5. Submit original Good Moral Character certificate with dry seal and Photocopied
6. Two pieces (2) Photocopied Birth Certificate (NSO)
7. If married, two (2) photocopies of marriage certificate duly signed by a priest / minister.
8. One (1) piece Long Brown envelope
Admission Requirements - Foreign Students:
1. Duly notarized letter of intent to study in National College of Science and Technology (NCST)
2. Six (6) original copies duly signed Student Personal History Statement (Form available at the Office of Student Affairs), containing his left and right thumb mark and 2×2 photo on plain white background taken not more than six months prior to submission
3. Scholastic Records authenticated by the Philippine Embassy/Consulate located in the student’s country of origin or legal residence
4. Photo copy of data page of passport showing photo, date and place of birth, date of issue and expiry authenticated by the Philippine Embassy/Consulate located in the student’s country of origin or legal residence
5. Birth Certificate or it's equivalent authenticated by the Philippine Embassy/Consulate located in the student’s country of origin or legal residence
6. Notarized Affidavit of support to cover expenses for the student’s accommodation, subsistence, school dues and other incidental expenses
7. Authenticated Police Clearance from the country of origin. (For someone who has resided in the Philippines for more than 59 days, an NBI clearance is also required)
8. Quarantine Medical Examination by the National Quarantine
9. Two (2) letters of recommendation from the Principal, Adviser, Counselor or any official from school graduated
