1. Original Card (Form 138)
2. Certificate of Good Moral Character
3. Three (3) 1×1 ID pictures w/green background
4. Photocopy of Marriage Certificate for married female students
5. Photocopy of Birth Certificate from NSO
6. CBC, Urinalysis, Hepatitis Screening (HBS), Chest X-Ray & Physical Examination conducted by NCF Physician (additional requirements for Nursing and Health Sciences)
7. Certification that the applicant belongs to the upper 50% of the graduating class, with a general average of 85% (additional requirement for Education)

2. Honorable Dismissal or Transfer Credential
3. Certificate of Good Moral Character
