Admission Requirements - Freshmen:
1. Original form of 138-A/High School report Card
2. Certificate of Good Moral Character
3. Photocopy of Authenticated NSO Birth Certificate
4. 4 copies of latest 2x2, colored I.D. Picture with white background
5. Must be physically and mentally fit and must be free from any of the following diseases: Epilepsy, Seizure Disorders, Hepatitis A, B and C, Heart Diseases, and any illnesses/diseases that may affect the student's performance in any setting.
6. Accomplished Student Covenant, Affidavit of Acknowledgement and School Policies and Individual Inventory Record
Admission Requirements - Transferees:
1. Transfer Credential Certificate
2. Recommendation of Good Moral Charater signed by the Dean of Student Services of the school previously attended.
3. Certified True Copy of Grades/Transcript of Records
4. Four (4) copies of latest 2x2, colored I.D. Picture with white background
5. Authenticated NSO Birth Certificate
6. Authenticated NSO Marriage Certificate, if married
7. Must pass evaluation and interview of the Program Coordinator
8. May be required to take written and oral qualifying examinations as well as physical and psychological examinations.
9. Must be physically and mentally fit and must be free from any of the following diseases: Epilepsy, Seizure Disorders, Hepatitis A, B and C, Heart Diseases, and any illnesses/diseases that may affect the student's performance in any setting.
10. Male student must passeed the required National Services Training Program (NSTP) or it's equivalent, for every semester it has to be taken.
11. Letter of Appeal address to the President duly notarized by a lawyer.
12. Police and NBI clearances
13. Accomplished Student Covenant, Affidavit of Acknowledgement and School Policies and Individual Inventory Record
14. Sworn Statement of Academic Condition (for BS Nursing)
Admission Requirements - Second Coursers:
1. Official Transcript of Records
2. Certificate of Good Moral Charater
3. Letter of Appeal address to the President duly notarized by a lawyer.
4. Police Clearance
5. Authenticated NSO Birth Certificate
6. Authenticated NSO Marriage Certificate (if married)
7. 4 copies of latest 2x2, colored I.D. Picture with white background
8. Letter of Permission from the employer, if working
9. Satisfactorily meets the cut-off grade of 83% in the entrance examinations.
10. May be required to take written and oral qualifying examinations as well as physical and psychological examinations.
11. Age requirement: not more than 35 years old
12. Evaluation of subject done by the Academic/Department Head.
13. Must be physically and mentally fit and must be free from any of the following diseases: Epilepsy, Seizure Disorders, Hepatitis A, B and C, Heart Diseases, and any illnesses/diseases that may affect the student's performance in any setting.
14. Behavior becoming of a student
15. Accomplished Student Covenant, Affidavit of Acknowledgement and School Policies and Individual Inventory Record
