Admission Requirements - Freshmen:
1. Original Copy of Form 138 (High School Card Report)
2. MMSU College Freshman Admission Test (CFAT) Rating Reports
3. College Admission Slip
4. Medical Examination Permit
5. Birth Certificate (issued by National Statistics Office (NSO)
6. One (1) 1”x 1” ID picture
Admission Requirements - Transferees:
1. Transfer Credential Eligibility or Honorable Dismissal
2. Certificate of Grades on courses taken; General Weighted Average must be at least 2.5
3. MMSU CFAT Rating Report for those who transfer with less than 30 units of credit
4. Certificate of Good Moral Character from the Guidance Counselor or Dean of previous institution attended.
5. Birth Certificate issued by National Statistics Office (NSO)
6. Validation of courses taken in a private institution where the grades are below 2.75 or 80%.
Admission Requirements - Foreign Students:
Undergraduate Admission Policies /Requirements For Foreign Students
1. Duly accomplished application form for undergraduate admission with a non-refundable application fee of $30 in money order or manager's check payable to MMSU.
2. Two original copies of the Official Transcript of Records (TOR) from each high school and college attended duly authenticated by the Philippine Foreign Service Post (PFSP) located at or having consular jurisdiction over the applicant's country of origin or place of residence. If these records are written in a language other than English, complete official translations must be submitted together with a copy of the original. The Transcript of Records should indicate all subjects taken and the grades earned. The grading system should be clearly stated on the TOR.
3. Duly authenticated copy of birth certificate and passport by the Philippine Embassy.
4. Two original copies of a notarized affidavit of support of the applicant's parents or Notice of Grant for Institutional Scholars to cover expenses of the student while studying.
5. Six (6) original copies of Personal History Statement (PHS) duly signed by him both in English and in his national alphabet and stamped with his personal seal, if any, and containing among others his original 2"x2" recently taken photograph with plain white background and his original left and right hand thumbprints. PHS forms may be obtained from any Philippine Mission abroad.
6. Result of English Proficiency Test for applicant whose native language or whose medium of instruction in secondary school is not English; a minimum score of 250 in the Test of English as a Foreign Language (TOEFL) or a rating of 4 in the English Language Test (ELT) is required. If the applicant did not take the TOEFL or IELTS, he may opt to undergo the crash program in English offered by the University.
7. A Filipino who graduates from a secondary school abroad applying for admission to the University must satisfy the same requirements as those of foreign students.
8. Course syllabus, school catalogues or description of courses taken from other colleges (for transferees).
9. Other institutional requirements:
• College Freshman Admission Test (CFAT) Rating.
• Certificate of Good Moral Character from the Principal or the head of the institution last attended.
• Two passport-size colored pictures with white background.
Graduate Admission Policies/Requirements For Foreign Students
1. Duly accomplished application form for Graduate School admission with a non-refundable application fee of US $ 50 in money order, cashier's or manager's check payable to MMSU.
2. Recommendation from two former professors or one professor and one immediate supervisor, if working.
3. Two original copies of the official Transcript of Records (TOR) from each college attended duly authenticated by the Philippine Foreign Service Post (PFSP) located at or having consular jurisdiction over the applicant's country of origin or place of residence. If these records are written in a language other than English, complete official translations must be submitted together with a copy of the original. The Transcript of Records should indicate all subjects taken and the grades earned. The grading system should be clearly stated on the TOR.
4. Duly authenticated copy of birth certificate and passport by the Philippine Embassy.

6. Six (6) original copies of Personal History Statement (PHS) duly signed by him both in English and in his national alphabet and stamped with his personal seal, if any, and containing among others his original 2"x2" recent photograph with plain white background and his original left and right hand thumbprints. PHS forms may be obtained from any Philippine Mission abroad.
7. Result of English Proficiency Test for applicant whose native language or whose medium of instruction in college is not English; a minimum score of 500 in the Test of English as a Foreign Language (TOEFL) or a rating of 6 in the English Language Test (ELT) is required.

9. Certificate of passing the Graduate Entrance Examination (GEE)
10. GPA of at least 2.00 and it's equivalent in a Bachelor's degree for a masteral program and at least 1.75 or it's equivalent in a masteral degree for a doctoral program.

2. Recommendations from two/three former professors or supervisors (GS Form 2) to be submitted by them personally or through mail to the Graduate School;
3. Original copy of transcript of records from the last school/educational institution attended and original copy of birth certificate
4. General weighted average of 2.5 or better in the BS or 1.75 in the master's degree for admission to the master's or doctorate degree, respectively;
5. A non-refundable application fee;
6. Two copies each 2" x 2" and one 1" x 1" colored pictures with white background; and
7. A “passed" rating in the Graduate Registration Examination (GRE).
Over and above these requirements, foreign students should submit the following documents:
1. A copy of birth certificate/authenticated passport;
2. Two copies of the official transcript of records written in English authenticated by the Philippine Embassy in the country of origin;
3. Permit to study issued by the Bureau of Immigration;
4. Four copies of personal history statement duly signed by the applicant written in English and in his/her national alphabet and stamped with his/her personal seal, if any, and containing among others his/her original recent 2" x 2" colored photograph with white background and his/her original left and right thumb marks;
5. Two passport-size colored pictures with white background;
6. Documentary proofs of support to cover expenses incidental to his/her studies (e.g. Bank statement, scholarship/fellowship grant);
7. Authenticated police clearance certificate issued by the national police authorities in the country of origin;
8. Medical Health Certificate (HIV, drug, etc.) issued by an authorized practitioner in the country of origin;
9. Student visa; and
10. Certificate of passing the Test of English as a Foreign Language (TOEFL) or English Language Test (ELT).

This school is included under the "Universal Access to Quality Tertiary Education" law that passed in 2017.
The law covers students enrolled in State and Local universities/colleges in Bachelors and Certificate programs.
Under the Law students in accredited State and Local universities/colleges will not pay any tuition fees or misc fee.
The Law covers only first time students (students who don't already have a college degree).
However, the full implementation of this law is not clear yet.
