Admission Requirements - Freshmen:
1. Form 138 (Original Copy of un-cancelled 4th year high school report card)
2. Form 137-A (Original Secondary Permanent Record)
3. Certificate of Good Moral Character issued by H.S. Guidance Counselor/Principal
4. NSO issued Birth Certificate
5. Entrance Examination Result
Admission Requirements - Transferees:
1. Certificate of Eligibility to Transfer (Honorable Dismissal)
2. Proof of grades, with course title and credit units
3. NSO issued Birth Certificate
4. Certificate of Good Moral Character from the Dean/Administrator of the previous school
5. For Filipino students who studied abroad, a copy of the Transcript of Records/Scholastic Records duly authenticated by the Philippine Foreign Service Post (PFSP)/Philippine Embassy in their country of origin must be submitted.
Admission Requirements - Foreign Students:
1. Transcript of Records/Scholastic Records duly authenticated by the Philippine
Foreign Service Post (PFSP) located in the student-applicant’s country of origin or
Legal residence
2. Photocopy of the student’s passport showing date and place of birth and birth
Certificate or it's equivalent duly authenticated by the PFSP
3. Student Visa or SSP to be applied by LCC Bureau of Immigration representative
NOTE:
Student Visa and Special Study Permit (SSP) from the Bureau of Immigration shall not be required of the spouses and unmarried dependent children below 21 years old of aliens under the following categories:
A.) Permanent foreign residents
B.) Aliens with valid working permits under Section 9 (d), 9 (g) and 47 (a) (2) of the
Philippine Immigration Act of 1940, as amended
C.) Personnel of foreign diplomatic and consular missions residing in the Philippines and their dependents
D.) Personnel of duly accredited international organizations residing in the Philippines and their dependents
E.) Holders of Special Investor’s Resident Visa (SIRV) and Special Retiree’s Resident Visa (SRRV)
F.) Foreign students coming to the Philippines with 47 (a) (2) visas issued pursuant to existing laws, e.g. PD 2021
