Course description:
The Bachelor of Science in Medical Laboratory Science degree program integrates the fields of life science and medicine with practical laboratory experience in clinical and public health settings. Medical Laboratory Science students develop organizational skills, quantitative reasoning ability and diverse laboratory science competencies for professional practice through focused coursework and hands-on experience in diagnostic laboratory medicine and public health. Upon completion of 3 ½ years of academic requirements, students will then proceed to their 6-month clinical internship and they have rotational duties in different sections such as Clinical Chemistry, Hematology, Microbiology, Immunohematology (Blood Banking), Immunology and Serology, Urinalysis and other Body Fluids (Clinical Microscopy), Parasitology, Histopathologic/Cytologic techniques and other emergent technologies at the different hospital affiliations of the College.

Course description:
The Bachelor of Science in Physical Therapy is a 5-year degree program that aims to develop competencies in different practice settings through patient/client management which includes examination, evaluation, diagnosis, prognosis, intervention and outcomes assessment. The program is composed of four (4) years of classroom instruction and one (1) year Clinical Internship Program composed of 1,700 hours of clinical exposure. Students are required to pass a Revalida Examination upon completion of the internship program prior to graduation. After graduation, students are required to take the Licensure Examination given bi-annually by the Board of Physical and Occupational Therapy of the Professional Regulatory Board. Potential employment opportunities include; clinical/hospital/institutional settings, sports, education, research, health and occupational safety, health and well-being and community-based rehabilitation.

Admission Requirements - Freshmen:
1. Original Grade 12 Senior High School Card
2. Original F137 (with request letter from OLFU)
3. Original Certificate of Good Moral Character issued in the current year.
4. Two (2) copies-recent identical 2”x2” studio photo (white background)
5. Clear photocopy of PSA Birth Certificate; for female, if married, a clear photocopy of Marriage Certificate
6. Clear photocopy of Grade 12 Senior High School Diploma
Admission Requirements - Transferees:
1. Accomplish the Application Form.
• Admissions Staff issues Student No.
• Pay the application processing fee of P220.00 at the Cashier’s Office.
2. For evaluation (credit subjects from previous school/s), submit any one of the following documents:
• Scholastic Records
• Certificate of Grades
• Class Cards
• Transcript of Records
Evaluation sheet is signed by both the Evaluator and Applicant. Original copy remains in the student envelope; carbon copy to applicant.
3. Pay the medical exam fee:
(a) P500 for all courses, except Maritime Education. Proceed to the Fatima University Medical Center for (1) X-ray – 1st flr and (2) Laboratory – 2nd flr. Results of both will be forwarded to the school physician who will notify only applicants with negative results for medication.
(b) P900 for Maritime applicants. Proceed immediately to Ophtha and ENT clinics – 2nd flr – Fatima University Medical Center. Applicant waits for results of color perception test and audiometry which must be submitted to the Admissions Officer; then to X-ray – 1st flr & Lab – 2nd flr.
4. Requirements for Enrollment:
• Honorable Dismissal / Transcript of Records or Transfer Credentials
• One (1) copy 2’ x 2” most recent photo (white background)
• Clear photocopy of PSA / NSO birth certificate
• For married female, clear photocopy of Marriage Certificate
5. Interview with the Admissions Officer to include signing of Statement of Commitment and Conformity; Promissory Note and Waiver re: Documents (for incomplete requirements). Discuss also payment plan: (a) Full / Cash or (b) Down payment of P1,500 plus 2 or 3 installments.
6. Payment of Fees:
Cash Payment is accepted at the designated Admissions Office Cashier, Student Accounting Office or designated banks inMarulas, Valenzuela such as Unionbank, Metrobank, Alliedbank, Eastwestbank, BPI and Chinabank; Malinta – Landbank and Asia United Bank.
Personal Check Payment – proceed to the Student Accounting Office. On the back of the check, please write the (1) Name of Student (2) Student No. (3) College Program (4) Home Address & Contact Numbers (landline and/or mobile phones)
ENROLLMENT:
7. Present Payment Receipt to Admissions Office Computer Encoder, claim Registration Card and Class Schedule.
8. Proceed to the Admissions Office Validator, claim the admissions kit and sketch of college uniform. Redeem Credit Memo for discount, if any: Jun 15 - Jun 30 (1st sem) or Nov 15 - 30, (2nd sem) only.
Admissions process now completed, attend classes by following the schedule on the registration card. Orientation for New Students is scheduled on the first week of classes.
