Admission Requirements - Freshmen:
1. Two (2) 2x 2 recent, identical color pictures with white background (with printed name tag in front and signature at the back)
2. Two (2) recommendation letters from the Guidance Counselor and the Class Adviser (Please use DLSL Form)
3. High school grades from 1st to 3rd year (Please use DLSL Form)
4. Certificate of Good Moral Character using the official school letter head w/ dry seal.
5. Photocopy of NSO certified Birth Certificate
6. Photocopy of the College Scholastic Aptitude Test results (if CSAT Passer)
• Passing Rate 60 percentile rank
• BS Nursing Applicants 70 percentile rank
7. Photocopy of complete scholastic records duly authenticated by the Philippine Embassy in the applicant’s country of origin and Diploma. The grading system should be clearly stated in the transcript. If these records are written in a foreign language other than English, complete and official English translations must be submitted. (Filipino Citizen from Philippine/International Schools). Please present the original documents.
8. Original copy of certification from the HS Registrar stating that the student has never enrolled in any college/university (if already graduated in previous school year)
9. Photocopy of Philippine Educational Placement Test (PEPT) certificate indicating eligibility for admission to college (if PEPT qualifier)
10. Photocopy of NSO certified Marriage Certificate duly signed by a priest/minister (if married)
Admission Requirements - Transferees:
1. Two (2) 2x 2 recent, identical color pictures with white background (with printed name and signature at the back)
2. Two (2) recommendation letters from the Guidance Counselor and the Class Adviser (Please use DLSL Form)
3. Transcript of Records (Certification of Grades)
4. Honorable Dismissal/Transfer Credential
5. Certificate of Good Moral Character using the official school letter head w/ dry seal.
6. Photocopy of NSO certified Birth Certificate
7. Photocopy of NSO certified Marriage Certificate duly signed by a priest/minister (if married)
Admission Requirements - Foreign Students:
1. Academic and documentary requirements set by the CAO (please refer to admission procedures)
2. Photocopy of complete scholastic records duly authenticated by the Philippine Embassy in the applicant’s country of origin (Original should be submitted to the Registrar’s Office during enrollment). The grading system should be clearly stated in the transcript. If these records are written in a foreign language other than English, complete and official English translations must be submitted.
3. Photocopy of the photo, data and stamp of the latest arrival pages of the passport reflecting applicant’s valid stay (Original should be presented during application)
4. Certificate of Residence for Temporary Student
5. Letter of Intent from the applicant (state reason/s for applying at De La Salle Lipa)
6. Letter from the Guardian (a guarantee that the applicant is personally known)
7. Financial Documents that shows the capability to study in the Philippines
8. Alien Certificate of Registration (Paper-based) / ACR I-Card (Microchip-based)
9. Student Visa or Special Study Permit
