3. NSO/PSA Birth Certificate (original and photocopy) with a clear copy of the original birth certificate issued by the Local Civil Registrar
4. Medical Certificate (preferrably issued by a Government Physician)
5. Drug Test Result (from a DOH/DNSC-Accredited Clinic Laboratory or Hospital)
6. Good Moral Certificate (original)
7. Form 137-A (secure a request form at the Registrar's Office upon enrollment
8. 4 Copies of recent 2x2 picture (white background)
9. 2 Long Brown Envelopes

Minimum Requirements
A bachelor’s degree, or masters degree, from a recognized institution is required for admission to a masters program or doctoral program, respectively.
For an applicant to a masters program, he/she must a graduate of 4 year course from CHED recognized college or university with a minimum Grade Point Average (GPA) of 2.5 or better, and for an applicant to a doctoral program, an applicant should have at least a GPA of 1.75 or better in the masters program in a CHED’s recognized college or university.
Evidence of suitable background or it's equivalent in the degree sought.

This school is included under the "Universal Access to Quality Tertiary Education" law that passed in 2017.
The law covers students enrolled in State and Local universities/colleges in Bachelors and Certificate programs.
Under the Law students in accredited State and Local universities/colleges will not pay any tuition fees or misc fee.
The Law covers only first time students (students who don't already have a college degree).
However, the full implementation of this law is not clear yet.
