Admission Requirements - Freshmen:
1. Authenticated Birth Certificate
2. Form 138 (Report Card)
3. Certificate of Good Moral Character
4. 2 pcs. Of 2×2 ID picture (Should be latest)
Additional Requirements/Qualifications for the Admission to the Nursing Program:
1. PEPT passers (Note: It is a placement examination given by the Department of Education to over-aged students)
2. Should have an average grade of 80%
3. Latest laboratory examination results (Note: Should present chest X-ray, urinalysis, fecalysis, and CBC results)
4. Physical examination
5. Doctor’s recommendation
6. Marriage contract (for married applicants only)

7. Transcript of Records (TOR)
PROCEDURE
1. Proceed to the Registrar’s office and Fill out the application form.
2. Pay for the entrance examination fee at the Business Office.
3. Go to the Counseling and Psychological Wellness Center (CPWC/Guidance Center and Present your official receipt (OR) for the scheduling of entrance examination.
4. Take the scheduled exam. (Note: Bring your Official Receipt and Application Form)
5. Proceed to the Registrar’s Office. Present the test result and get the list of requirements.
6. Next, go to the school clinic to acquire a referral form for the laboratory exam.
7. Once done, proceed to the clinic to submit laboratory results for your physical and dental exam. (Note: Place the documents inside in a long yellow expanded envelope).
8. Proceed to the Registrar’s office and submit the requirements for checking.
9. Go to the scheduled interview and get a notice to enroll.

2. Pay the evaluation fee at the Business Office.
3. Proceed to the Registrar’s office to submit your TOR for the crediting of subjects and to determine the subjects to be enrolled in a certain semester.

2. NMAT score (Should be 40 or above
3. Honorable Dismissal
4. NSO Birth Certificate
5. General Weighted Average in College (GWA)
6. Letter of Recommendation from Dean or Faculty of previous school
7. Certificate of extracurricular activities (e.g. Civic/volunteer work)
8. Four (4) copies of 2x2 photo
9. Professional licenses (if any)
(For Master of Arts in Nursing)
1. Should have a Bachelor of Science (BS) in Nursing degree
2. Must have nursing work experience (Hospital Experience)
3. Take and Pass the Graduate School Entrance Exam
