Admission requirements:
A. ELIGIBILITY 1. BS or AB Degree - Should have graduated from college/university, or should be graduating from college/university at the end of the 2nd Semester SY2014-2015 - Should complete college/university requirements by May 2015 - Applicants who will complete college/university requirements beyond May 2015 can apply on October 2015 for SY2016-2017 2. Satisfactory performance in the following required courses (course descriptions below) - General Biology or General Zoology or General Botany (3 units lecture and 1-2 units laboratory) - Cell and Molecular Biology (3 units lecture) - Biochemistry (3 units lecture) - General Physics (4-5 units lecture and laboratory) 3. NMAT score (with a percentile rank of 90 or higher)
B. APPLICATION REQUIREMENTS 1. Read and fully understand the ASMPH Admissions SY2015-2016 Frequently Asked Questions (FAQ) HERE 2. Availability to attend the Transition Program in June to July 2015. (Non-attendance or unnecessary absences in this summer program may mean dismissal from ASMPH.) 3. Online Application: Available HERE (Read and fully understand the notes and instructions indicated in this Applicant Access Module.) 4. Application fee (in cash or check payable to: “Ateneo de Manila University” - Php 3,000.00 for local (Filipino) applicants - USD 300.00 for foreign (non-Filipino) applicants 5. Official Transcript of Records – Undergraduate, and, Graduate (if any) 6. Certification of General Weighted Average (GWA) or Quality Point Index (QPI), if not included in the Official Transcript of Records 7. Certificate of Class Ranking (based on GWA/QPI) 8. Official NMAT Results 9. Recommendation Letters (2) from the following: - Dean - College Secretary - Department Chairman - Senior Professor 10. Personal Essay (to be uploaded in the online application) Topic and essay guidelines are indicated on the online application. 11. 2” x 2” photo (to be uploaded in the online application) Photo guidelines are indicated on the online application.
NOTE: - Applicants who pass the document review will be scheduled for interview. - All incoming first year ASMPH students will be required to attend the Transition Program on June to July 2015. Non-attendance or unnecessary absences in this program may mean dismissal from ASMPH.

Admission Requirements - Transferees:

5. Photocopy of Unofficial Transcript of College Grades covering up to the last completed semester (Photocopy of Registration form with list of classes, class cards or computer print-out grades certified by the school may be submitted. Please bring original copies for verification.)
6. Scholarship/Financial Aid Questionnaire (if applicable)
7. (For international applicants except United Arab Emirates) Scholastic Aptitude Test I (SAT I) Results if replacing the ACET with SAT I. Note: The SAT I must have been taken within the last two years prior to the application.
International applicants who have downloaded the application form must enclose the application fee payment. Please see the payment schedule below:
International applicants except applicants from United Arab Emirates and Middle East - US$50 or Php2,800
Applicants taking the ACET in the United Arab Emirates - US$100 or AED 370
Applicants from the Middle East taking the SAT I in lieu of the ACET - US$50 or Php2,800
Payments (cash or cheque) must be in good condition (i.e., without creases, torn edges and pen marking). Foreign cheques must be from a major US bank or it's affiliated local banks. Please make cheque payable to “Ateneo de Manila University”.

A. For International Freshman Applicants (except United Arab Emirates and the Middle East)

2. High School Record and Principal's Recommendation Form
3. Two (2) sealed Recommendation Forms
4. Personal Essay
5. Acknowledgment Slip
6. Scholarship/Financial Aid Questionnaire (if applicable)
7. One of the following:
• Ateneo College Entrance Test (ACET) Exam Permit
• Scholastic Aptitude Test I (SAT I) Results. The SAT I must have been taken within the last two years prior to application.
• International Baccalaureatte (IB) Diploma Scores (Please attached a letter signifying the intention to use the IB Diploma for application).
8. Enclosed Application Fee US$50
1. One (1) photocopy of the filled-out Form
2. Two (2) copies of personal essay (topic is indicated in the application form)
3. Two (2) photocopies of the applicant’s official transcript of undergraduate and graduate records (if applicable)
4. Four (4) colored 2” x 2” size ID pictures
5. Three (3) duly accomplished Recommendation Forms
6. Two (2) photocopies of the applicant’s birth certificate
7. Two (2) photocopies of marriage certificate (for married women)
