Admission requirements:
1. A holder of a baccalaureate degree taken not more than (5) years prior to the filling of the application. For non-health related degrees, applicant must have minimum prerequisites of 15 units Biology, 10 units Chemistry, 5 units Physics, 6 units Mathematics, 12 units Social Sciences.
2. NMAT score of 60 or higher is preferred
3. Accomplished College of Medicine Application Form at http://myauplifebook.aup.edu.ph/v.2/aoa with receipt/proof of payment of Application Fee, refer to: http://www.aup.edu.ph/5/bank_details for payment details.
4. Certified True Copy of Transcript of Records and Certificate of Completion/Graduation in-cluding Over-all GPA.
5. Certificate of Good Moral Character
6. Four (4) copies of 2x2 recent photo
7. NBI Clearance
8. Copy of Birth Certificate on official NSO paper
9. Photocopy of Marriage Certificate, if applicable
10. Letters of Recommendation: two (2) from for-mer professors at last school attended and one (1) from your clergy or church leader. These must be in individually sealed envelopes sent directly and addressed to:

Admission Requirements - Freshmen:
1. Accomplished Application Form with 2” x 2” picture recently taken.
2. Original Form 138/High School Report Card
3. Photocopy of PSA Authenticated Birth Certificate
4. Photocopy of PSA Authenticated Marriage Certificate, (if married)
5. Payment of application, testing, ID and medical examination fees
6. Two Recommendation Letters/Forms filled up by (a) school principal/adviser, (b) district pastor/parish priest/barangay captain
7. Original Certification of Good Moral Character from the school last attended
8. Pictures: 5 copies (2” x 2”) recently taken
9. High school GPA of 85% and above for BSED/BEED/BSN applicants
10. Minimum GPA of 85% or 2.8 to be admitted to Dentistry Proper
11. Photocopy of high school diploma for BSN applicants
12. Certificate of Eligibility to Transfer from the last school attended (for transferees)
13. Certification of Grades/Transcript of Records (for evaluation purposes)
14. Certification from the principal that the applicant graduated valedictorian, salutatorian and first honorable mention from a class of 50 and above (for honor students)

7. Foreign Student’s Pledge (notarized)
8. A grade point average (GPA) of C (2.50, 80-82%)
9. Diploma/Degree Certificate (authenticated)
10. Police Clearance Certificate issued by the National Police Authority in the country of origin or residence (authenticated)
11. NBI Clearance for applicants who graduated in the Philippines
12. Medical Health Exam Results (authenticated) for the following:
- Urine
- Stool
- Chest X-ray
- Malaria Smear
- Venereal Disease Research Laboratory (VDRL)
13. Vaccination Certificate for polio and yellow fever
14. Birth Certificate (authenticated)
15. Certificate of No Objection (transferee)
16. Certificate of Eligibility to Transfer (transferee)
17. 2” x 2” picture (5 pcs.)
18. Copy of Marriage Certificate (if married) authenticated
19. Photocopy of Student’s Passport showing date and place of birth (authenticated by the Philippine Embassy in the country of origin)
20. Affidavit of Support (authenticated) or grant for institutional scholars
21. Bank Reference (authenticated) not less than US$5,000.
22. Payment of Fees:

- Identification Card (ID)
23. Financial Security Deposit (US$1,000) which could be withdrawn upon finishing the program or leaving the University permanently.
An applicant is required to take entrance tests in Mathematics, English, and Psychological tests to be administered by the University Guidance Office. If an applicant failed in the English test, he/she will be advised to enroll in English as a Second Language courses for one semester before he/she will be allowed to register in a regular class.
Additional Information for Foreign Students
In order for foreign students to have a pleasant stay in the country, the student must abide with the following instructions:
1. Upon arrival of the student in the Philippines, he/she is required to go to the National Quarantine Office for medical clearance. Then he/she proceeds to the Office of the Bureau of Immigration (BI) for application for ACR I-Card.
2. The student is expected to be a full time student, not less than 15 units and should finish the course within the time specified in the approved curriculum.
3. All foreign students are expected to stay in the dormitory and eat at the University Cafeteria which serves vegetarian food only. Married students with family must apply for student apartment.
