Admission Requirements - Freshmen:
1. Form 138 (Report Card)
2. Certificate of Good Moral Character signed by your school principal
3. Photocopy of NSO Birth Certificate
Note:
• Form 137 will be requested by the university registrar from your school principal.
• If you’re planning to enroll in the College of Criminology, submit the requirements listed above along with a medical certificate.
• If you’re a Handog Kaibigan grantee, submit the requirements listed above along with the following:
- Photocopy of Handog Kaibigan certificate/letter
- Certificate of Indigency authorized by your barangay
Admission Requirements - Transferees:
1. Certificate of Eligibility to Transfer (formerly known as “Honorable Dismissal”)
2. Transcript of Records or certified true copy of complete academic records or informative copy of credits and grades signed by your college/university registrar
3. Certificate of Good Moral Character signed by your college/university dean
4. Photocopy of NSO Birth Certificate

1. Study permit duly issued by the Bureau of Youth Affairs, Foreign Students and Foreign Schools and the Commission on Higher Education upon presentation of student visa or immigrant visa
2. Alien Certificate of Registration
3. Statement of Eligibility for Admission to a particular course in the Philippines
4. Immigrant Certificate of Registration
5. Three photocopies of your passport and visa
6. Three photocopies of Alien Certificate of Alien Registration issued by the Bureau of Immigration
7. Three recent 2×2 ID pictures
