Proceed to the Graduate School Office (2nd flr Graduate School Building, Main Cmpus) and accomplish
Application form for admission to the graduate school. Confer with the Dean and fill up enrollment adviser form. Submit to the Admissions office the following requirements:
1. Original and two photocopies of Transcript of Records
2. Two colored ID pictures, size 2x2
3. Certificate of good moral character by any of the following:
(a) school where bachelor degree was earned for those enrolling in masteral program and school where MA was earned for those enrolling in PhD program or
(b) certificate of employer if currently employed.
• above items to be submitted and placed inside 1 long brown envelope
For Transferees:
Transfer student applicants are required to present a copy of their undergraduate and graduate Transcript of Records. Graduate credits earned from other Philippine schools will be evaluated by the office of the Dean of the Graduate School - OLFU. Those earned from foreign schools will be evaluated by the Commission on Higher Education (CHED) upon the recommendation of the Dean of the Graduate School - OLFU. Submit to the Admissions office the following requirements:
1. Honorable Dismissal
