Admission Requirements - Freshmen:
1. High School Report Card (Form 138)
2. Form 137A (if available)
3. NCAE-National Career Assessment Exam (if available)
4. Letter of Admission (Entrance Exam Result)
5. Certificate of Good Moral Character (if available
6. Ishihara Test Result (for BSMT /BS MarE /SRC, Marine Courses only)
Admission Requirements - Transferees:
1. Transcript of Records or Certification of Grades or Class cards with Prospectus/Subject Description
2. Honorable Dismissal /Transfer Credential
3. Certificate of Good Moral Character
4. Letter of Admission
5. Ishihara Test Result (for Marine Courses)
Admission Requirements - Foreign Students:
1. Photocopy of Alien Certificate of Registration (ACR)
2. Photocopy of Student Visa or Passport•
3. Two (2) passport-size pictures
4. English Proficiency Test. In addition to the entrance requirements prescribed for Filipino citizens and those prescribed by the Commission on Higher Education, foreign students must pass an English Profiency Test to be given by the English Department of the university.

6. Must pass the entrance examination.
• Otherwise, admission letter from the Counseling and Testing Center.
1. Report to the Graduate School (GS) office for interview with the program coordinator and acquire an endorsement letter.
2. Proceed to the Counseling and Testing Center (CATC) to secure an examination permit. Pay the non-refundable entrance examination fee to the Cashier’s Office.
3. Take entrance examination on the scheduled day and submit your most recent 1×1 ID photo to the CATC.
4. Call up GS after 5 working days for the examination result.
5. After passing the examination, come back to the Graduate School Office to secure the enrollment form. Proceed to the Registrar’s Office to submit the following requirements:
• One (1) ID picture (1”x1”)
• Original Transcript of Records with Special Order Number
• Two (2) letters of recommendation
(from former professor AND from present employer)
